STEP 1. DETERMINE ACTIVATION LEVELS

Research shows that activated people are more successful self-managers. The question then becomes "How do we identify who is activated and to what degree?" Once we determine this, we can create tailored action plans that are appropriate to the individual. Health Coach provides these answers with a validated approach to identify activation levels. Health Coach engages and motivates participants to learn more about themselves through our unique personal health profile. This profile summarizes personal attitudes and health biases as well as providing a "reality check" that is motivational and inspires people to take action.

 

STEP 2. CREATE TAILORED ACTION PLANS

Based on a person's activation level, we provide a customized action plan and tailored learning program. By following the principles of live health coaching, we allow participants to prioritize what actions they feel are most appropriate, assist in removing barriers-to-change and track progress. Actions are based on the latest clinical practice guidelines so you can be confident that there is meaningful progress towards better health. Over time, participants will see their accomplishments, improve their confidence, increase their activation levels and become successful self-managers.
